Perform a factory reset on a Polycom device using the MAC address method, please follow the steps below:
Reboot the phone. As soon as the white screen appears with the message "Starting application", press and hold the 1, 3, and 5 keys simultaneously. When prompted for the admin password, enter the MAC address of the phone in all uppercase letters, without colons or spaces.
To switch between input modes during password entry:
1 → Aa: Numbers
A → a1: UPPERCASE letters
a → 1A: Lowercase letters
You can usually find the MAC address on the back of the phone or on the original box label.
